BC Provincial Blood Coordinating Office is a program of the Provincial Health Services Authority, with offices at 300 - 1867 West Broadway, Vancouver, British Columbia, V6J 4W1 (herein "PBCO") Tel:  604-714-2888.


By using this Transfusion Medicine Data Portal (herein "Data Portal"), you agree to the terms and conditions that governs the use of this Data Portal. If at any time, these terms and conditions are no longer acceptable to you, you must immediately cease all use of this Data Portal.


The information in this Data Portal (the "Information") is provided "as is", for informational purposes only and may contain inaccuracies and typographical errors. PBCO makes no representation as to the correctness, quality, or reliability of the Information and disclaims all express and implied representations, warranties, and conditions of any kind, including but not limited to representations, warranties, or conditions regarding accuracy, currency, timeliness, sequence, completeness, non-infringement, merchantable quality, and fitness for any particular purpose. Any reliance you may place on the Information is at your sole risk.

PBCO reserves the right to delete, modify, or supplement the Information and other content of this Data Portal at any time for any reason, without notification.

PBCO does not warrant that this Data Portal, or the information contained herein will be uninterrupted or free from defects or errors, nor that this Data Portal and its associated servers are free from computer viruses or other harmful content. No endorsement of any third party products, services or information is expressed or implied by any information, material or content referred to or included on, or linked from or to this Data Portal. Nothing contained in the Information should in any way be construed as being either official or unofficial policy of the PBCO.

The Data Portal has been specifically developed for use by the BC transfusion medicine community as well as the Ministry of Health.

Conditions of Use

In consideration of being granted personalized access to this Data Portal, you agree:

1. That you are solely responsible for maintaining the security of your Password and ensuring that it is used for yourself only;
2. To change your password at least every 42 days;
3. That if you suspect that your password has been compromised to change it immediately and notify the PBCO;
4. Not to write down your User ID and password, store it in a file on your computer or permit anyone to observe you entering your User ID and password;
5. That your access to the Data Portal is through the Health Authorities' secured IT network;
6. To restrict access to the Data Portal to use only within Canada.
7. That your use of the Data Portal will be monitored and recorded in an audit log and reviewed regularly.

You acknowledge that your Password is unique to you and that PBCO does not have access to your Password. PBCO is not responsible for unauthorized access to accounts online or losses that occur as a result of you voluntarily disclosing your username and password, or the careless or improper handling, storing or disclosure by you of this information.

For additional protection, after 5 unsuccessful login attempts a user will be locked out. Users are automatically logged out after 30 minutes of inactivity.

PBCO may suspend or terminate User Access immediately without notice if it is determined that you have undertaken activities deemed to be inappropriate. This suspension will be for a period of time necessary to permit the thorough investigation of such suspended activity.

PBCO is not responsible in any manner for direct, indirect, special or consequential damages, however caused, arising out of your use of this Data Portal and/or any web browser.

Transfusion Medicine Data Portal Privacy Statement

We are committed to maintaining confidentiality and personal privacy. Collection, use, disclosure and retention of information are subject to, and must comply with, the provisions of the BC Freedom of Information and Protection of Privacy Act (FIPPA). The purpose of this privacy notice is to describe our practices regarding information use and collection through this Transfusion Medicine Data Portal (herein "Data Portal"). Please note that we do not disclose, give, sell or transfer any information about our Data Portal users unless required for law enforcement or otherwise provided for by law.

We reserve the right to update this policy at any time. Users are responsible for reviewing this document periodically for changes.

What kind of information is being collected and how is it used?

This Data Portal does not capture or store personal information such as your name or home address while you visit and use the Data Portal. The Data Portal is intended only for internal users within the Health Authorities' secured IT network. All users will access it from a Health Authority site or hospital, or using a Health Authority authorized remote access mechanism, such as a token. The Health Authority's IP address is collected.

Information collected automatically (Cookies):

Some of our web pages use "cookies" which are small amounts of information stored by your web browser software on your work station. Cookies are designed to track browsing history on a website generally for the convenience of the user. They help the website "remember" what areas of a website a browser has accessed or used previously. Cookies can, however, be misused, and have become a privacy concern for some because they can be used to record and analyze an individual's broader browsing history.

The cookies we use on this Data Portal and what we use them for are described below. This Data Portal operates on 'implied consent', which means that by visiting and clicking through the Data Portal, we assume you agree with this usage. If you do not agree, then you should not use this Data Portal, or you can delete the cookies after visiting this site, or use your browser's anonymous usage setting (called "Incognito" in Chrome, "In Private" for Internet Explorer, "Private Browsing" in Firefox and Safari). It's important to know that cookies are plain text files and cannot execute files or make copies of themselves. They do not carry or deliver viruses and cannot scan or retrieve your personal information. There are two types of Cookies – Session and Persistent.

Session Cookies

Session cookie information is deleted as soon as you leave the Data Portal. The use of temporary session cookies is solely for the purpose of allowing the user to interact with the Data Portal in an effective manner.

In a few cases our Data Portal may use persistent cookies to store information for longer periods than the current session.

Monitoring and Logging

Use of the System will be monitored and recorded in an audit log, which will be reviewed regularly to ensure compliance with the Terms of Use. The following is some of the information collected as you browse through our Data Portal:

• IP address (internal to Health Authorities' secured IT network).
• The date and time of each user visit.
• Which reports were run during the session.

The Data Portal was developed to support the BC transfusion medicine community in quality, utilization and other decision making needs. The Data Portal is a web-based tool designed to empower stakeholders with the ability to pull their own TM data in a secure and efficient manner to better support decision making.

There are three components to the Data Portal:

1. Dashboard: Displays a selection of data "widgets" that provide a visual snapshot of recent blood product utilization data trends.

2. CTR Reports: Allows users to run or export established Health Authority and Provincial level CTR reports in a variety of formats.

3. Cubes: Displays information in a crosstab format that allows the user to filter and break down data into different perspectives.
