Error Management
In order to support compliance with the Health Canada blood regulations, PBCO collaborated with the BC Patient Safety Learning System (PSLS) to revise the Blood Transfusion or Blood Product - Safety Event Report Form to include events related to the BC Red Cell Redistribution Program.
Typically, issues involving red cell redistribution are identified and reported by the Receiving Site but need to be investigated and followed up by the Shipping Site.
Reporting errors and accidents through PSLS facilitates opportunities for continuous improvement. Health Authorities can also use the PSLS report functionality to comply with the Health Canada annual reporting requirement.

Redistribution Programs
Red Cells and Factor Products account for over 40% of the cost of blood components and products in British Columbia (BC). To help minimize waste, the BC Provincial Blood Coordinating Office (PBCO) coordinates two provincial programs.

Factor Redistribution
Factor products are used primarily for the prevention and treatment of bleeding episodes in patients with hemophilia or other congenital/ acquired bleeding disorders. Factor products are expensive blood products with a shelf-life of two to three years. In order to minimize product expiry and waste of unused hospital stock, the BC Provincial Blood Coordinating Office (PBCO) coordinates a provincial Factor Redistribution Program. The safety and quality of redistributed factor products are ensured by the adherence of participating hospitals to the factor redistribution procedures.
BC hospitals send near-expired (<6 months from expiry date) stock to St. Paul’s Hospital in Vancouver. The St. Paul’s Hospital transfusion service works with the BC Hemophilia Program to ensure that the redistributed product is used prior to expiry.
Factor products to be redistributed to St Paul’s Hospital include:

- - Human Factor VII (7)
- - Factor VIII (8), Recombinant Factor VIII and Long Acting Factor VIII
- - Factor VIII (8) von Willebrand Factor Complex (HumateP and Wilate)
- - Porcine Factor VIII (8)
- - Factor IX (9), Recombinant Factor IX and Long Acting Factor IX
- - Factor XI (11)
- - Factor XIII (13)
- - Prothrombin Complex Concentrate (Octaplex, Beriplex)

BC Red Cell Redistribution
To minimize red cell waste, the BC Provincial Blood Coordinating Office (PBCO) coordinates a provincial Red Cell Redistribution Program. In this program, small and medium-sized hospitals send blood that is 10-14 days before expiry to designated large hospitals where the blood is more likely to be used. The BC Red Cell Redistribution Program helped the province achieve a provincial red cell expiry rate of 0.64% in 2021-22.
Participating hospitals help to maintain the safety and quality of redistributed red cells by adhering to the policies, processes and procedures described in this manual. The PBCO thanks all participants in the BC Red Cell Redistribution Program for their contributions and effort to conserve this precious resource.
